Somehow gets carried into the real world when he awakens. He still has the drawings collected, the notes and the knife, although I believe he carries that round everywhere anyway. (Nutter...). Anyone hazarding any explanations for this?

How do we know the real world is actually real? Silent Hill's presence fucks up any reality. Alex most likely never leaves the otherworld, even when it's back to 'normal' Shepard's Glen is still infested with monsters.
Alex has also been in an asylum so we don't know if any of those drawings actually exist or are a figment of his imagination.

But wait, doesn't he lose the flashlight though? 'Cause he picks it up again in the house, on the bottom bunk in his room. So only the health and notes and whatnot are carried over.
See, I have a theory about this sort of thing, when game characters start carrying around way more than they should be able to: I call it the Pocket of Leaves phenomenon. See, all these characters have a special pocket somewhere on them that they can put an unlimited amount of stuff in, then pull it out at any time. In Alex's case, his Pocket of Leaves also has special trans-dimensional/trans-consciousness properties, so the stuff he picked up in his "nightmare" sequence stays with him when he wakes up. As for why he has to get a new flashlight, it's because the flashlight he had in his dream is poking out of a simple, regular pocket, and thus at the end of the dream is rendered insubstantial.

Indeed, so maybe they did think about it! Nice notice And since he is a soldier, having a knife is kind of normal i guess.

Vinny boy, this is a Silent Hill forum. We don't need any more House of Leaves Refferences in here than we already have.
Ahem... Right, personally I have a different Belief. If you got the Hospital ending first time (which I did, thank you very much), or any time for that matter... hell, any ending for THAT matter... you know Alex is a Nutjob Braincase. Personally I believe that his time in the Asylum, ahem... hadn't ended by the time the game begins. And the Hospital Ending? Canon.
So the Nightmare and then Afflicted Shepherds Glen? All part of the same Delusion.
In unrelated matter, I am wrighting a [gayvoice]Fanfiction[/gayvoice] about what would happen when Alex gets out of the Asylum... And I won't post it here, because I don't believe in eating where you ____.
